The Department of Health (DOH) Kontra Covid platform is a web-based application where users can register health information and conduct self-assessment to check your risk of infection as a service to respond to the COVID-19 pandemic in the Philippine setting.
Children, aged 10 - 17 years old, are required to seek consent from parents or guardian in providing personal information through this application.
Our Privacy Notice
This DOH Kontra Covid platform in partnership with GovSolutions Inc., collects the following information from you when you submit to us your personal data record as seen in the form you have completed. This platform shall collect personal and sensitive personal information to ensure proper selection, management, and analysis for the purpose of public health monitoring and to manage appropriate response. This platform shall collect the following:

It is important that we collect accurate information about you. Why do we collect your personal data? The Department of Health, as the Personal Information Controller (PIC), shall use your personal data to properly receive volunteer inquiries, forward them to appropriate internal units for action and response, in a legitimate format and in an orderly and timely manner.
By accessing the web application, you share some personal information that will be processed for the purpose of monitoring and analyzing web traffic. This will allow us to maintain and improve our services for the benefit of DOH. The information processed may include but is not limited to IP addresses, system configuration information, and other information about traffic to and from the webapp, devices, and/or networks (collectively, “Log Data”).

Do we share your personal data to other institutions or organizations?
Yes, the platform shall have two (2) other Personal Information Processors (PIPs), namely:
- GovSolutions Inc. (AI4GOV)
- DOH Regional Epidemiology Surveillance Units
with whom we may share your information solely for the purposes of data collection, organization, communication, and management; in accordance with the RA 10173 or the Data Privacy Act. Together, the PIPs enable the DOH to properly address inquiries, forward them to appropriate internal units for action and response, in a legitimate format and in an orderly and timely manner.
How do we secure your personal data?
Only authorized DOH personnel as well as staff from the above-mentioned data processors will have access to the personal information on a need-to-know basis. These data will be stored in a secured database in accordance with the period provided by DOH. All personal and sensitive personal information about you will be destroyed along with its erasure from the database, or in line with either the individuals upon DOH’s decision. All personal and sensitive personal information processed by the DOH and the authorized personal information processors shall be kept under strict confidentiality for the entire period of retention.
