The Department of Health (DOH) Kontra Covid platform is a web-based application where individuals can register health information and conduct self-assessment to check their risk of COVID-19 infection. This serves as a public service tool to actively respond to the pandemic in the Philippine setting.
Children, aged 10 - 17 years old, are required to seek consent from parents or guardian in providing personal information through this application.
Our Privacy Notice
This DOH KontraCOVID platform in partnership with GovSolutions Inc., collects regular and sensitive personal information to provide provisional health risk assessment, appropriate advice, and forwarding to local government health units with active access accounts for data validation and proper response. This platform collects the following information from you when you submit to us your personal data record as seen in the form you have completed:

By accessing the web application, you share some personal information that will be processed for the purpose of monitoring and analyzing web traffic. This will allow us to maintain and improve our services for the benefit of DOH. The information processed may include but is not limited to IP addresses, system configuration information, and other information about traffic to and from the web application, devices, and/or networks (collectively, “Log Data”).

Do we share your personal data to other institutions or organizations?
Yes, the platform shall have three (3) Personal Information Processors (PIPs), namely:
- GovSolutions Inc. (AI4GOV)
- DOH Epidemiology Surveillance Units
- Local Government Units
with whom we may share your information solely for the purposes of data collection, organization, communication, and management; in accordance with the RA 10173 or the Data Privacy Act. Together, the PIPs enable the DOH to properly address inquiries, forward them to appropriate internal units for action and response, in a legitimate format and in an orderly and timely manner.
How do we secure your personal data?
Only authorized DOH personnel as well as staff from the above-mentioned data processors will have access to the personal information on a need-to-know basis. Your personal data will have standard encryption in the application and stored in a secured database in accordance with the period provided by DOH. All personal and sensitive personal information about you will be destroyed along with its erasure from the database, or in line with either the individuals upon DOH’s decision. All personal and sensitive personal information processed by the DOH and the authorized personal information processors shall be kept under strict confidentiality for the entire period of retention.

Know your Rights
Our data handling operations are guided by the RA 10173 or the Data Privacy Act. Know more about your rights through the National Privacy Commission via https://www.privacy.gov.ph/know-your-rights/.
You have the right to access your personal information. You have the right to rectify your information. If your data is inaccurate or incomplete, you may do this by accessing your personal information through the link that will be shared upon submission. Your information is secured and can be accessed by using a one time password (OTP) that is sent to your registered mobile number.
